To become a paid IHSS provider in California you must complete the provider orientation, submit a SOC 426 enrollment form, and pass a criminal background check via Live Scan fingerprinting. CDSS won't activate your provider number until your prints clear — typically a few business days after we transmit them.

Bring the 8016 you received at IHSS provider orientation — not a generic one. The form's "Type of Application" and "Requesting Agency" fields tell us how to bill the submission.
